How do I create a form with a data field on the form, the data for this field could come from data blocks (#1 or #2)/ 2 tables. 

Just like "select MyDate from tableA if ... then select YourDate from tableB else tableB"

make two data items for seperate block and place one over another

set the visible property of data item according to your condition on runtime
