Are oracle forms and reports 10g compatible with oracle database 12c ?

what are the prerequisites for oracle database upgrade from 10g to 12c.

There is no reason I am aware of why your old Forms will not work with the current database. You can check the detail on MOS, on the Certifications tab.
The database upgrade is straightforward, all described in the Upgrade Guide, http://docs.oracle.com/database/121/UPGRD/toc.htm
