Please, help me add flexfield to a form

I have made all steps from articles below

But window for input FlexField values does not show

Problem is resolved and it was caused by wrong short_application_name in FND_DESCR_FLEX.DEFINE(.
It needs to use Application Short Name which mention in form "Descriptive Flexfields".
I used Application Short Name with which i have registered my table.
