"FRM-92101: Unexpected Server Failure "

occurred due to misconfiguration on the server side.
this error can occur when

CAUSE: JInitiator uses the browsers proxy settings

ACTION: Go to Control Panel JInitiator 1.3.x.x proxies and deselect the Use

Browser Settings and enter the details for the proxy settings.

CAUSE: The specified working directory does not exist. This can be confirmed by looking
for a log message like Unable to switch to Working Directory "workingDirectory"

in the application log file the application log file can be found at

application-deployments/formsapp directory of the OC4J instance on which Forms is deployed

ACTION: Edit the forms configuration file to correct of the working directory.

CAUSE: If the forms applet parameter Heartbeat is set to a value more than
FORMS_TIMOUT.

ACTION: Make sure that the value of heartbeat is less than FORMS_TIMEOUT
